Adaptation of Montefiore Exercises for Home-Based Cardiac Rehabilitation, with a Personalized Focus on Heart Failure

Abstract

Heart failure is a terminal condition whose management requires cardiac rehabilitation to improve quality of life. The case of a 72-year-old woman with heart failure and chronic coronary syndrome, who underwent valve replacement and coronary revascularization, is presented. She was enrolled in a home-based cardiac rehabilitation program following the Montefiore guideline. After ten weeks of individualized exercises, she showed improvement in the stress test, reduced heart rate, reduced respiratory quotient, and increased peak oxygen consumption (+1.3 METs). These results reflect a lower risk of hospitalization and mortality, with improved functional and aerobic capacity. It is concluded that Montefiore exercises applied in the home setting are an effective, accessible, and beneficial intervention for patients with heart failure.
